Evansville Integrative Medicine is now a member of the St. Mary's Physician's Network. As such we can now accept many forms of insurance. However, some of the modalities of treatment that we offer are not covered by the majority of insurance companies. More and more insurance companies are providing coverage for acupuncture, however, most still do not. Both acupuncture and out-of-network medical care are eligible expenses if you have a medical savings account or a medical spending account through your employer. Reiki and some other modalities may not be covered by any insurance policy.

We will file insurance claims, including Medicare, for you, for covered services. You are responsible for filing your own claims for reimbursement for acupuncture. You may request documentation of your visit that you can submit to your insurance company for reimbursement, as allowed by the particular plan. Please note: because insurance plans differ, the amounts of reimbursement differ. No guarantees or predictions can be made on how much of the fee will be covered by each company, or whether it will be covered at all. It is the patient or the patient's representative's responsibility to determine whether reimbursement is available and in what amount. Please advise your insurance company that the services will be provided by an MD, this will result in the highest rate of reimbursement to you.

We will be happy to obtain prior approval from your insurance carrier for any recommended tests and procedures and to refer you to laboratories or providers, including specialists, who are in your insurance carriers' network. If prescription medications are recommended, we will be happy work with you to attempt to find appropriate medications that are on your insurance carrier's preferred drug list.

Initial Visits for Integrative Medicine
No two patients are alike and no two initial appointments are alike. You will receive a package of materials from us to complete prior to your first appointment. There will be an extensive health and social history questionnaire. I will review this with you at our first appointment. You will also receive a thorough physical exam. Laboratory tests may be ordered, if indicated. I may then ask you to return in 1-2 weeks to review my findings and the lab results with you.

On-time Office Policy
One of my personal frustrations with physicians' offices is that they don't run on time. In some offices it is not unusual to wait an hour or more to see the doctor. I value your time and my time more than that, and I run an on-time office. I schedule PLENTY of time for us to talk as my appointments are available in 25 or 50 minute increments. No other patients are scheduled in your appointment time. Because of this, your appointment starts at the scheduled time and ends at the scheduled time. If you are late, it is your time that is wasted.

Treatment Related Questions
It is my goal to be of support to you in your progress toward wellness. If you have a question, concern or feedback related to your current treatment (e.g., need for clarifications on recommendations, unusual but not alarming sensations, problems ordering medications or supplements), please call the office between 9 AM and 5PM Monday through Friday. If I am not available at the time of your call, I will get back to you as soon as possible. I accept after hours phone calls for minor emergencies. In return, I ask that you please consider whether your question can wait until normal business hours before calling.

If at any time you have concerns that are of an emergency nature or that are alarming you, such as allergic reactions, chest pain, trouble breathing, significant headache or similar, please call 911 or go straight to the nearest Emergency Room.

Test and Lab Results
We will call you with your lab results.  No excuses. No “If you don't hear from us, everything is fine” silliness. You deserve to know what the results are. I will tell you. If you want the results mailed or faxed to you, we will mail or fax you copies.  If you have questions, I'll be happy to answer them. I'll send your results anywhere you want me to (as long as I either have your verbal permission or a signed release of information). It's your body, it's your health, and I wouldn't have ordered the tests in the first place unless we both really wanted to find out something. That means they're important.  That means you should get the results and an explanation of what they mean. 

Confidentiality Policy
I don't release anything to anyone, under any circumstances, unless I have your signature on a release of information and unless I or my staff have validated it with you in person or on the phone. (The only exception is if you TELL ME, either on the phone, or in person, to go ahead and release something, but then I’ll be following up with a formal release of information to document that in every case.)  I don’t blindly accept “blanket releases” that insurance companies send out.  So don’t be surprised if you get a call from us if you have signed one, making sure that we have your permission to send the information. 

Cancellation and No-Show Policy
All cancellations and appointment changes must be made no less than 24 hours prior to the appointment time. Appointment times are not overbooked or double booked as in many physicians' offices. This allows us to see you promptly at your scheduled appointment time, but means that it is difficult to fill a last-minute vacancy in the schedule. Consequently, cancellations made less than 24 hours prior to the appointment time and all "no-shows" will be charged the full price of the appointment. Cancellation calls should be made to the office at (812) 401-4222.

Please note that telephone calls to remind you of a scheduled appointment are a courtesy. You are responsible for knowing when your appointment is scheduled regardless of whether you receive a reminder call.

* Notes on fees- Established integrative medicine patients wishing to receive acupuncture pay only the session fee for acupuncture, no initial visit fee is charged.
